Special Note for parents or guardians of minors receiving service: We take your minor's health and experience to be our top priority. Fortunately for you, so does the state of Florida. Please carefully READ the additional minor information sheet in the release form. Then, PRINT the notary form included in the packet and obtain a notary's signature and seal before your service appointment. Notary forms must be completed on paper, we can not accept electronic copies.
